sex-based differences in serotonin distribution. Men have more in the left frontal lobe, womenâs is more evenly distributed. Women donât retain brain serotonin levels as well as men without eating foods containing tryptophan, which the body converts to serotonin. Serotonin levels drop precipitously during a migraine attack, as much as 60 per cent. Weâre trying to fix wherethe serotonin activity occurs, what triggers the drop, all getting back to the fundamentals of what exactly initiates migraine.â
âGive me a sec,â Claire said, scribbling. âDid she say who the piece was for?â
âProbably. I donât remember. LâAube might. Itâs true we never heard back from her but thatâs not unusual with journalists.â
âWas there anything particular in what you said that seemed to catch her attention, that she might have wanted to follow up on?â
âWe talked a little about drugs, too. The failure of current triptan drugs to cross the blood-brain barrier and thus affect whatâs going on in the brain stem. Thatâs the goal, ultimately.â
âTo cross the blood-brain barrier.â
He nodded.
âHow close are you to that?â
âWell, first we have to understand the mechanism.â
âDid you give her any other leads? People to contact?â
âThereâs the guy at Sunnybrook in Toronto. Some good people in the UK.â
He gave her names and she wrote them down. âDid she happen to mention that she gets migraines?â
âShe mentioned it. Not auras but pretty frequent, severe, kind of drug resistant by the sound of it.â
âWould you happen to know if she had a migraine when she came to see you?â
âAt one point she said so but she wasnât obviously incapacitated.â
âDid she seem in any way upset or on edge?â
âNo, but she wanted something I wasnât prepared to give her. We do these studies, examining serotonin levels in patient volunteers during the course of a migraine. You have to have not taken any drugs in the last twenty-four hours and we put you through a PET scan. She said she hadnât taken anything and was willing. But she wouldnât be in town for any kind of follow-up, and thatâs part of the study. And thereâs a preliminary interview. Weâre not set up for walk-ins. Though she was very persuasive.â
âSo what happened?â
âI told her, under the existing protocols, I couldnât use her in the study. That was pretty much it.â
âWhen was this?â
âAround three, four in the afternoon. Hang on, Iâll check the date.â He picked up the phone and dialled. âPierre, a quick one. Got a woman here whose sisterâs disappeared. Journalist, here in March. Yeah, I talked to her. You spoke to her by phone. Okay. Itâs in your book?â He looked at Claire. âMarch 15. Three p.m.â Hung up the phone. âYou get them, too, donât you?â
Which took her aback. âYeah. Do you?â
âNo, go on, hate me. You seem to know the language. Thereâs a physiological predisposition that runs in families, as Iâm sure you know. Mother? Grandmother?â Claire nodded. âYours as bad as your sisterâs?â
âNot quite as bad.â
âThe usual tyramine-based triggers â chocolate, red wine, et cetera, et cetera?â
âOther ones, too.â
âThe drugs work for you?â
âMostly.â
He rose to his feet. âI hope some of this helps. I hope you find her. If you have any more questions, let me know.â
